Community Support & Wellbeing

Our Community Support & Wellbeing Team will work with you to get you on a positive and individual path to resolving the challenges you face. Whether that is at home, in your community, GP surgery or in hospital.
Our Aim is to support people in East Lothian to:

  • Access local and national support services, so their journey through services are smoother and more efficient

  • Help people and communities support themselves through a variety of self-management, peer and volunteer led initiatives. 

  • Build capacity in communities through partnership working

  • Recognise the importance of person centred and outcomes focussed support and care planning, ensuring people are involved in their care and support journey

  • Reducing inequalities, break the cycle and impact of deprivation and support and protect the vulnerable in our communities – Positive Personal Outcomes

The Community Support Service offers local people and their families the opportunity to help the elderly in their community.



As a Community Health Champion you will ensure that our elderly neighbours do not stay in hospital any longer than necessary.
